The Support Broker Program coordinates supports and services to individuals with developmental disabilities and traumatic brain injury.
As participants live their lives and change how they want to be supported, we ensure a person-centered approach is used in service planning to establish self directed supports.
The philosophy of the Support Broker Program is to provide services that assist in developing the self-sufficiency of individuals, and to protect, preserve, and promote dignity of all persons. Our belief is that people with developmental disabilities and traumatic brain injury, and their families, have both the ability and the right to make decisions that influence their lives.
A Support Broker is the person selected by an individual with developmental disabilities or traumatic brain injury to help obtain desired services.
Who Is Eligible

Adults with developmental disabilities and traumatic brain injury

Service is available to Dane County residents and as determined eligible by the Dane County Department of Human Services
Services Offered

Help develop and facilitate annual Person-Centered Plan with the individual, family, and invited team members

Offer support as people live their lives and change how they want to be supported

Coordination of activities, counseling and placement

Providing information on service options and available community resources

Identifying potential issues of health or safety

Case management services including service coordination

Assistance to individuals to achieve their personal goals

Advocacy on behalf of individuals for financial benefits and funding increases for changing support needs
