Program Success
Abby is a shy, non-verbal participant at Community Connections. When she first began participating at Community Connections she needed a lot of encouragement and had reportedly never made eye contact.

However, over the past year Abby made great strides in expressing herself. Community Connections holds recreational activities and enjoys regular visits from a musician called “Country Bob”. When Bob plays his guitar and sings Abby smiles, claps, and dances in her seat. She is frequently observed with a smile that lights up her face as she watches the activity around her or takes part in the many activities that are offered to her.

Community Connections gives adults with developmental disabilities or traumatic brain injury the opportunity for daily community integration and activity.

Community Connections promotes the diverse and unique abilities and interests of each participant. Consumer independence and choice are emphasized whenever possible.

Our staff is committed to providing the participants with quality care in a respectful and safe environment. Knowledge and compassion are combined to provide individualized supervision and training to those who attend the program.

Who Is Eligible

  • Individuals with a developmental disability (mental retardation, autism, epilepsy, cerebral palsy, traumatic brain injury)
  • Service is available to residents of Rock County and as determined eligible by the Rock County Developmental Disabilities Board

Services Offered

  • Community-based and facility-based service program for adults with developmental disabilities
  • Programming is offered weekdays in a safe, well-supervised environment where high support needs and challenging characteristics of the participants are best served
  • Staff to client ratio of 1:3 and 1:4
Community Connections Office
Community Connections provides adults with developmental disabilities the opportunity for daily community integration and meaningful activity. We serve clients from 8:00 a.m. to 3:00 p.m., Monday through Friday. Clients are assessed for admission to Community Connections following a recommendation by their case manager.

Our building was custom-designed with our clients and their needs in mind. The rooms at the Community Connections office are painted with warm colors, and are open and inviting. We have ample space, a large kitchen, and four ADA-approved wheelchair accessible bathrooms. The Wisconsin Room, River Room and Cafe are our three great rooms where most in-house activities take place. They are furnished with comfortable, accessible seating and dining furniture so that we can be cozy in whatever activity we're doing.
