Social Worker/
Developmental Disability Case Manager

Position Summary
Join a case management team and an agency that offers competitive wages and excellent benefits in a supportive environment. We are seeking to fill a full-time position in our Rock County case management program. Position involves providing case management services and support to adults with developmental disabilities. Duties include completing annual assessments and developing outcomes for each individual on the caseload. Other key duties include the coordination and monitoring of residential and vocational placements, medical services, financial services, and service costs.

Minimum Qualifications
Qualifications needed include an MS or BS in Social Work or Human Services field. Previous experience providing case management and/or support to individuals with developmental disabilities is also required. Candidates must also have top-notch skills in organization, verbal and written communication, and time management.

Weekend and Back-up Youth Support Specialists 

Position Summary
Catholic Charities, Youth Living Alternatives Program, has weekend and back-up positions available providing support to adolescents with severe, emotional disturbance, autism, and/or other developmental disabilities. Back-up positions may include awake overnight as well as evening and weekend shifts. Back-up positions must be willing to have a flexible schedule and be available to work on short notice.

Duties include but are not limited to: assisting with and teaching personal cares and activities of daily living, following a behavior support plan, providing opportunities to participate in community and social activities, completing paperwork and household duties.

Minimum Qualifications
Previous experience working with challenging behaviors and knowledge on autism spectrum disorder is a must.  Must be able to adhere to written protocols and procedures.

Assistant Program Director

Position Summary
Excellent management position in our Supportive Living Program serving individuals with developmental disabilities and traumatic brain injuries. This is a full-time, salaried position working 37.5 hours per week. Responsibilities include but not limited to department-wide recruiting, interviewing, hiring and orientating direct care staff, assisting in disciplinary meetings, completing monthly paperwork per county requirements, assisting with quality assurance measures, supervising on-call positions and supervising manager in the Program Director's absence. If you like high intensity in a fast paced environment, this job is for you!

Required skills include outstanding organizational skills a must; excellent verbal and written communication; exceptional time management skills; prior supervisory experience; prior experience working with individuals with developmental disabilities and traumatic brain injuries in a community setting; strong recruiting skills within the field of developmental disabilities; experience working with challenging behaviors; work effectively as a member of a multi-disciplinary team; comply with all agency, county, and state paperwork, annual reviews, and other requirements; knowledge of dual diagnosis a plus.

Minimum Qualifications
Master's Degree in Social Work or related field, supervisory experience, strong recruiting skills within the field of developmental disabilities, and experience working with individuals with developmental disabilities in community settings.

This position requires the use of a personal car. Mileage is reimbursed at a current rate of $0.51 per mile.

Community Support Specialist
Flexible Placement – “Floater”

 

Position Summary
Catholic Charities’ Supportive Living Program has an immediate opening working with people with developmental disabilities and traumatic brain injuries. Involves providing assistance with daily living activities and recreation.

A minimum of 15 hours per week is guaranteed with the potential for up to 25 more, depending on changing needs for filling in for others.

Minimum Qualifications
Requires maximum flexibility and readiness to report to work with as little as 4-6 hours notice. Also requires readiness to be trained to support a broad variety of clients needs in multiple households. Paid training.

Behavioral or CAN experience preferred, but not required. Must have a car.

Catholic Charities Supportive Living Program has rewarding positions available providing support to persons with traumatic brain injury and developmental disabilities. Saturday and Sunday morning, afternoon, and evening hours available  immediately.  Assist with activities of daily living and recreation. Excellent benefits available for 30+ hours per week.

Position Summary
Become part of a nationally accredited program serving seniors in their home in Dane County.  Catholic Charities is seeking part-time candidates for a Companion position working with senior adults. Responsibilities: providing relief for family caregivers, companionship, and transportation, preparing meals, light housekeeping and may include personal care.

Position Summary
